Assessment Description

Applying a model or framework for change ensures that a process is in place to guide the efforts for change. In 500-750 words, discuss the model or framework you will use to implement your evidence-based practice proposal project. You will use the model or framework you select in the Topic 8 assignment, during which you will synthesize the various aspects of your project into a final paper detailing your evidence-based practice project proposal. Include the following:

Identify the selected model or framework for change and discuss its relevance to your project.
Discuss each of the stages in the change model/framework. Describe how you would apply each stage of the model or theoretical framework in your proposed implementation.
Create a concept map for the conceptual model or framework you selected to illustrate how it will be applied to your project.
Attach this as an appendix at the end of your paper. Refer to the “Evidence-Based Practice Project Proposal – Assignment Overview” document for an overview of the evidence-based practice project proposal assignments. You are required to cite minimum of four peer-reviewed sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.

Complete the “APA Writing Checklist” to ensure that your paper adheres to APA style and formatting criteria and general guidelines for academic writing. Include the completed checklist as an appendix at the end of your paper. Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. The model or framework that I have selected to use for my evidence-based practice proposal project is the Stages of Change Model (also known as the Transtheoretical Model). This model is relevant to my project because it provides a structured approach for understanding and guiding behavior change, which is essential for the implementation of an evidence-based practice.
The Stages of Change Model consists of five stages: precontemplation, contemplation, preparation, action, and maintenance. Each stage represents a different level of readiness for change and requires a different approach for promoting behavior change.
In the precontemplation stage, individuals are not aware of the problem or are not ready to make a change. In this stage, it is important to provide education and raise awareness about the problem and the benefits of making a change.
In the contemplation stage, individuals are aware of the problem and are considering making a change. In this stage, it is important to provide support and resources to help individuals weigh the pros and cons of making a change.
In the preparation stage, individuals have decided to make a change and are preparing to take action. In this stage, it is important to provide information and resources to help individuals plan and implement the change.
In the action stage, individuals have made a change and are actively working to maintain it. In this stage, it is important to provide ongoing support and resources to help individuals sustain the change.
In the maintenance stage, individuals have successfully made and sustained the change. In this stage, it is important to provide ongoing support and resources to help individuals maintain the change over time.
I would apply each stage of the Stages of Change Model in my proposed implementation by identifying which stage the target population is in, and then tailoring my approach to match the specific needs of that stage.
I have attached a concept map as an appendix that illustrates how the Stages of Change Model will be applied to my project. This concept map shows the different stages of change and the specific strategies that will be used to promote behavior change at each stage.
The Stages of Change Model is a suitable framework to use for my evidence-based practice proposal project. By identifying the specific stage of change that the target population is in, I can tailor my approach to match the needs of the population and increase the chances of successful implementation.
