Mr. G is a 66-year-old Hispanic gentleman who presents to the clinic with a chief complaint of bilateral lower leg pain. He first noticed the pain earlier this week while he was cleaning out his garage. Mr. G. describes the pain as, “persistent over the last few days at a 5 or a 6 when getting up in the morning”, based on a 1-10 scale. He feels less pain when walking (at a 4 or a 5) and feels even less pain when he is seated with his legs propped up, but he prefers to be active. A review of his medical record reveals a history of hyperlipidemia, hypertension, “diet-controlled” type II diabetes and former smoker (quit more than 10 years ago). His current daily medications are Atorvastatin 20 mg, benazepril 40 mg, metoprolol 25 mg, and a “baby aspirin.” Current vital signs are BP 154/94, HR 92, RR 20, o2 stats 96% on RA. Based on an exam, bilateral lower extremity findings are as follows: slightly bluish color, visible varicosities, +1 edema ending just above ankles, capillary refill brisk, skin temperature warm, and peripheral pulses strong.
