The Integrated Assessment Report assignment consists of four parts @ 100 pts each. To establish a solid understanding of the scope of the project, please read the directions for all parts before you begin.
• Part 1: In Part 1 of the assignment, you will administer a standardized comprehensive achievement test covering the main academic domains of reading, writing, and mathematics to an appropriate and consented volunteer. You will score the test and interpret the findings.
• Part 2: In Part 2 of the assignment, you will administer a criterion referenced achievement test in reading, or writing, or mathematics to an appropriate and consented volunteer. You will score the test and interpret the findings.
• Part 3: In Part 3 of the assignment, you will administer a behavioral/social emotional measure to an appropriate and consented volunteer. You will score the test and interpret the findings.
• Part 4: In Part 4 of the assignment, you will integrate the results of the 3 administered tests listed above (standardized, criterion, and behavioral/Social emotional) into a integrated summary report. The integrated report should include the following 10 sections:

Assignment Overview:
You will plan, administer, and score 3 assessments to an appropriate consented volunteer. Then based upon the test results, you will write an integrated assessment report, using the template provided. 5. What are some recommendations for modifications, instruction, intervention, or strategies based upon the test results?

Part 2
Part 2 Guiding Notes:
Administer one criterion referenced achievement test:
1. Select a criterion referenced test (performance based).
2. Administer the criterion referenced test to your volunteer.
3. Score the criterion referenced test based upon the appropriate scoring criteria.
4. Interpret the test score results – what were the strengths and weaknesses of the student volunteer?
5. What are some recommendations for modifications, instruction, intervention, or strategies based upon the test results?

Part 3
Part 3 Guiding Notes:
Administer one behavioral/social-emotional test:
1. Select a behavioral/social emotional test (BASC-3, Conner, etc). Choose a technically sound instrument that minimizes bias.
2. Administer the behavioral/social emotional test to your volunteer.
3. Score the behavioral/social-emotional test based upon the appropriate scoring criteria.
4. Interpret the test scores – what were the strengths and weaknesses of the student volunteer? Any areas of concern?
5. What are some recommendations for modifications, instruction, intervention, or strategies based upon the test results?

Part 4
Part 4 Guiding Notes:
Integrate the assessment results findings into a single report.
1. Using the report template provided, complete each of the 10 report sections.
1) Demographic Information -
2) Reason for Referral - Establish rationale for assessment and how the information will be useful. What are the specific questions and concerns about this child?
3) Assessment Procedures- List the multiple types of assessments administered. Be sure the assessments selected are appropriate and demonstrate knowledge of diversity in culture, development, and learning.
4)Background Information – Discuss essential information about the child’s history, status, and circumstances. This information is gathered through collaboration and communication with school personnel, families, and the student. Be sure to apply knowledge of diversity issues.
5) Behavioral Observations- Observations should be low inference and pertinent to the referral question and provide insight into the child’s functioning.
6) Assessment Results- Presented accurately and in an understandable and meaningful manner.
7) Clinical Impressions- Consistent with findings from the multiple assessment sources
8) Summary- Concise, highlights key findings
9) Recommendations -Integrate and interpret your assessment findings. List recommendations to meet your volunteer’s needs, interests and learning styles. Be sure to include academics, behavior, personal and social development, daily living, and occupations/ careers. Also be sure to include collaboration between school personnel.
10) Data Summary- includes all scores, clearly labeled Tables, along with essential related data (e.g., percentiles, confidence intervals) and interpretive guidance
