Individuals’ ability to seek and obtain medical care when they need it is critical to their health and the health of their community. For a variety of reasons, access to health care may be restricted. Health disparities and access to health care can be exacerbated by socioeconomic factors. According to the Healthy People 2030 initiative, social determinants of health, of which health care access and quality is one of the five components, can influence the quality of an individual’s well-being (health.gov,2022). According to Nuako et al (2022), access to healthcare is limited for people who live in rural areas, where there are fewer providers, and who have limited access to transportation. According to Nuako et al, 2022, other reasons included an inability to afford medications and a lack of insurance. Another reason patients were unable to see a provider was a scarcity of providers in the areas, particularly in rural areas. As a result, patients’ appointments were not scheduled on time. According to Nuako et al. (2022), this could have contributed to diagnoses that could have been avoided.
2.From the Healthy People website, choose an objective related to your assigned social determinant. Determine the objective’s status and the population to which it applies.
People in Good Health AHS-08’s goal is to increase the proportion of adults who receive recommended evidence-based preventive healthcare.
This goal is aimed at adults. According to Healthy People, this goal has not improved since it was established (Health.gov,2022).
2.Select an objective related to your assigned social determinant from the Healthy People website linked above. Identify the status of the objective and the population to whom the objective applies
Healthy People Objective: Increase the proportion of adults who get recommended evidence-based preventive healthcare- AHS-08
This objective is aimed at adults. According to Healthy People, this objective has not shown any improvement since it was initiated (Health.gov,2022).
3.Provide at least three appropriate questions related to the chosen objective that the NP could incorporate into the health history and interview to assess the client and family?
To provide evidence-based preventive healthcare to adults as recommended by Healthy people Initiative, the NP can incorporate the following 3 questions into the health history and interview that will aid in assessing the client:
1. What is the specific clinical need of this client?
2. Where can the highest quality of answers come from?
3. Is the information obtained sufficient?

CDC.gov
Clinical Practice Guidelines contain recommendations on how to treat or manage a patient diagnosis. The recommendations are made by experts in that field and are usually evidenced-based knowledge. The CDC website is a government provided resource that has up to date information on the disease, prevention, management and other resources that can aid providers in making the best decisions regarding patient care.
