Clinical Case Formulation Format The best way to formulate / construct the paper is to use the directions as a template. For example, use sections 1-5 as headings (e.g. Background with Demographics, Assessment of the Client, Theoretical Orientation, etc.). Under each header use subheadings (e.g. MSE, DSM-5-TR Diagnosis, Risk, etc.). The use of APA headings and subheadings helps guide the reader through the document, gives the reader a general idea of what to expect from the paper and leads the flow of discussion. These elements divide and define each section of the paper. The paper should include 8 to 10 pages of content, plus a cover page and reference page. Use a minimum of 4 additional scholarly references, in addition to your textbook. Scholarly references can include peer-reviewed articles, journals, government websites and included supplemental resources. Cite all information from the required resources/textbook plus the 4 additional scholarly references using proper APA style. Use the information in the rubric as a guide toward successful completion of the assignment. The paper format is not a DO A CLIENT MAP nor should use bullets.

Client is a 32-year-old African American woman who is self-referred due to depressive symptoms including reported moods of “helplessness”, “anger”, and “sadness”. Client expressed that her bottled in emotions come from current and past family occurrences that involve regularly being disrespected and feeling that they plot against her when she is not around. Client has two children ages 7 and 2 years old, who she recently received a Child Protective Services (CPS) report regarding with the allegations of psychological harm. Client expressed that she felt that she needed to talk to someone due to everything going on in her life because she felt that she was losing herself as well as her children. Client reported that she is the oldest of five children. Client stated that when she was 12 years old, she entered foster care due to repeated physical abuse from her mother and her mother’s boyfriend. Client stated that she was later placed in the custody of her great uncle and his wife in a loving environment. Client stated that she felt she needed to talk to someone about everything that was going on in her life because she felt she was losing herself and her children. She is the oldest of five children, according to the client. Client stated that she entered foster care when she was 12 years old due to repeated physical abuse from her mother and her mother’s boyfriend. Client stated that she was later placed in the loving custody of her great uncle and his wife. Client stated that once she was older, she felt that she needed to build a relationship with them but has since regretted doing so. Client reported that her mother and siblings constantly manipulate her in to feeling that she did something wrong for having a better life than them and always want something from her. The client stated that every time she tells them that she cannot do something for them, they will call in a CPS report on her. The client’s primary DSM-5 diagnosis is posttraumatic stress disorder, F43.10. The client also has a secondary DSM-5 diagnosis of acute stress disorder with depressed mood, F43.21.
