1. Examine this case study through the dependency cycle model
(Fig. 14.3). The outer arrows show a progression through varying
stages of dependency. The inner circle represents who can be
involved in the dependency cycle. Where are Jane and Dan in this
cycle?
2. Using the basic dependent-care system model (Fig. 14.4), assess
Dan and Jane. Identify the basic conditioning factors (BCFs) for
each. What is the effect of Dan’s BCFs on his self-care agency? Is
he able to meet his therapeutic self-care demands? Continue on
to diagnose Dan’s self-care deficit and resulting dependent-care
deficit. Now assess Jane’s self-care system.

Examining the Case Study Through the Dependency Cycle Model
To address your first question, Jane and Dan can be located within the dependency cycle model shown in Figure 14.3. Based on the details provided, Dan currently finds himself in the dependent stage of the cycle, as he is relying on Jane for his daily care needs due to his recent injury. Jane has taken on the dependent-care agent role in caring for Dan, placing her in the caregiving stage.
Assessing Dan and Jane Using the Basic Dependent-Care System Model
Using the basic dependent-care system model in Figure 14.4 as a framework, we can further analyze Dan and Jane’s situation. For Dan, his basic conditioning factors (BCFs) that influence his ability for self-care include his recent injury, which has physically impaired him. This has negatively impacted his self-care agency, as he lacks the ability to independently meet his therapeutic self-care demands on his own at this time without Helpance.
For Jane, her BCFs include familial obligations as Dan’s partner, as well as likely work and financial responsibilities. While she has taken on the role of dependent-care agent for Dan, this additional caregiving workload has potentially increased her own stress levels and affected her ability to adequately meet her own self-care needs.
To diagnose Dan’s condition, he exhibits a self-care deficit due to his physical impairment negatively impacting his self-care agency. This then results in a dependent-care deficit, as he relies on Jane to Help him with his daily care needs. For Jane, providing care for Dan has caused a dependent-care demand that has yet to be fully addressed and could potentially start affecting her own self-care if not properly managed.
Designing a Nursing System to Support Jane’s Increased Caregiving Role
To help address Jane’s dependent-care demands as she cares for Dan, a nursing system could be designed with the following components:
Conduct a home visit assessment to evaluate Jane’s self-care abilities and dependent-care demands in order to create an individualized care plan (Bucher et al., 2020).
Provide education on stress management techniques and setting healthy boundaries to prevent caregiver burnout (Family Caregiver Alliance, 2016).
Arrange periodic respite care to allow Jane temporary breaks from her caregiving responsibilities (Evercare, 2007).
Make referrals to community resources like meal delivery, transportation, or home health aides that can provide additional support (AARP, 2016).
Follow-up regularly to reassess needs and adjust the care plan as necessary (Adelman et al., 2014).
