Ms. Cook is a 28-year-old woman who was referred to your mental health agency by her case manager at Free From Drugs, Inc. She is currently on probation for shoplifting, passing bad checks, vandalism, and parole/probation violations. She has been charged four times with disorderly conduct, once for fishing without a license, and twice for driving without a license (she never applied for one).  Ms. Cook has been incarcerated twice during adulthood (once for 10 months and, most recently, for 10 days). After leaving jail recently, she has been staying with her sister, who wants her to move out as soon as possible.

She describes her present physical health as “good. She has never received psychiatric care. During your assessment, Ms. Cook tells you she has been in out-patient treatment for dependency problems associate with a substance use disorder.  She currently uses crack cocaine two times a day, except on the days she goes to Free From Drugs, Inc.  She wants to stop using cocaine and get a job, but on the days she tries not to use she develops feeling of intense fear of going out and avoids job interviews. She avoids social situations and interacting with others. She says she feels her muscles tense up at the thought of going on a job interview. These feelings prevent Ms. Cook from being able to start the process of applying for work, and makes being around friends and family unbearable.

 

1) Formulate a DSM 5 diagnosis based on the case content.  Include ICD-10 numeric coding, official wording for the disorder, and any specifiers that may be applicable.  Consider using the diagnostic categories of Other Conditions That May Be a Focus of Clinical Attention (detailed in the DSM-5, pages 715 -727).

2)  Provide a narrative justification for your diagnostic selection.  You may use your DSM 5 for this assignment.
