Community Teaching Plan

The purpose of this assignment is to plan and develop an outline for your community teaching plan and gain feedback from a community health representative in your local community. The teaching plan developed in this assignment will be used to develop your Community Teaching Presentation due in Topic 5. Complete the following:

Identify the demographic that you would like to give your presentation to and then select a community site where that demographic can be found (Community name “ Woodford Community “ Cedar Park, TX)
Complete the “Community Teaching Acknowledgement Form,” found in your topic Resources. This form requires a handwritten signature from the appropriate representative selected, allowing permission for you to teach your selected topic at that site.
Complete the “Community Teaching” template resource in your topic Resources. This form will help you organize your teaching plan and create an outline for the written assignment due in Topic 5.
Review the plan with the representative that signed the “Community Teaching Acknowledgment Form” and request feedback (strengths and opportunities for improvement). The feedback received will be used to refine the teaching plan in Topic 5.

Please note: For this assignment, you will submit two attachments: the “Community Teaching Acknowledgement Form” and the “Community Teaching Plan” proposal. Be sure to review “Submitting Assignments,” found in the topic Resources, for tips on submitting multiple files in one dropbox.

The “Community Teaching Experience Approval” form is a clinical document that is necessary to meet clinical requirements for this course. Therefore, the acknowledgment form should be submitted with the provider’s hand-written signature. A typed, electronic signature will not be accepted.

You are required to cite a minimum of three sources to complete this assignment. Sources must be published within the last 5 years, appropriate for the assignment criteria, and relevant to nursing practice.

APA style is not required, but solid academic writing is expected.

Identification of Focus for Community Teaching:
The focus of the community teaching will be to educate adults on the impact of environmental issues on their health and the health of their community.

Epidemiological Rationale for Topic:
Studies show that environmental pollutants contribute to numerous health problems such as respiratory diseases, cardiovascular disease, and cancer. In the urban community, exposure to pollutants is high due to the high concentration of industrial and transportation activities.

Goal: Healthy People 2030 (HP2030) objective utilized as the goal for the teaching is:

Rationale: To address the high exposure to environmental pollutants in the urban community and reduce the health risks associated with exposure.

Behavioral Objectives:
