Create a patient registry for your selected population group that you outlined in your Wk 2 – Summative Assessment: Patient Registry Proposal. Ensure your patient registry collects the appropriate data as defined by the patient registry it will report to and ensure it is searchable.
Using Microsoft Excel®, design a spreadsheet to collect all relevant data elements for this database. Each data variable must have its own dedicated column, along with a standard format for recording each data point. Among the data variables you are building into the database, include patient identifiers for your planned method of patient matching. These can vary depending on the type of patient registry you are building, but the primary concern should be on patient privacy and security.
Include a data dictionary on the first tab of your Excel file spreadsheet that defines the following using standardized language, coding, formatting, ranges, and values:
The basis for registry population’s inclusion and exclusion criteria
The data elements for each column and its specific format – ranges and allowable values for each individual data element
The information requested from patients to produce the data, if data is qualitative
The laboratory range values, when applicable
Which data elements are critical to registry’s stated purpose
Use the data from the patient registry you identified in Weeks 1 and 2 to populate your database with the data from at least 50 patients.
