Reflecting on the “IHI Module TA 102: Improving Health Equity,” describe two causes of health disparities in the United States, or in your local community, that lead to health inequity. What ethical issues inhibit access and quality for care for these issues? Outline an initiative, integrating your faith and ethical principles surrounding practice, to reduce these health inequities and sustain the change within the health care system.
DNP- Patient Outcomes And Sustainable Change
Patient outcomes refer to the results or consequences of healthcare interventions or treatments on the health and well-being of patients. Patient outcomes can be measured in a variety of ways, including through changes in symptoms, quality of life, and survival rates.

Sustainable change refers to lasting or permanent improvements in healthcare systems or practices. In order to achieve sustainable change, it is important to identify and address the root causes of problems and to involve all stakeholders in the planning and implementation process.

Improving patient outcomes and achieving sustainable change in healthcare can be challenging, but it is important for the overall effectiveness and efficiency of the healthcare system. By focusing on evidence-based practices, continuous quality improvement, and patient-centered care, it is possible to make positive and lasting improvements in patient outcomes.
