Patient showed improvements in mood, which was the main objective to achieve this week. The patient's mood has continually improved for the past three weeks. The symptoms of depression have decreased, as per patient’s reports crying less, and improving her sleep time. The patient was encouraged to continue with follow-up psychotherapy. Individual psychotherapy and follow up scheduled next week to continue improving the patient's condition.

The patient is a 28-year-old male who presented with chief complaints of excessive worry, trouble sleeping, and irritability. In today's consultation session, the patient appears alert and oriented to person, place, time, and situation. He was cooperative and engaged throughout the session, with relevant and coherent speech. The patient reports experiencing persistent anxiety symptoms despite his medication and regular psychotherapy. His mood is anxious, and his affect is congruent with his mood. Thought processes are normal, and the thought content is within normal limits. The patient has shown some improvement in sleep and decreased irritability with medication and psychotherapy but still reports significant worries that interfere with daily functioning. The patient was encouraged to continue with regular follow-up appointments and to practice relaxation techniques between sessions to help manage anxiety symptoms.
