CHF patients are at high risk of hospital readmission, which can lead to increased healthcare costs, decreased quality of life, and even death. The aim of this paper is to examine the rationale for readmissions among CHF patients and provide evidence-based interventions for reducing hospital readmissions in this population. Congestive heart failure (CHF) is a chronic condition that affects millions of individuals worldwide. According to the World Health Organization (WHO), approximately 26 million people are affected by CHF globally, with the majority of cases occurring in high-income countries. In the United States alone, an estimated 6.5 million individuals have CHF, and it is the leading cause of hospitalization among individuals over the age of 65.
