In community and public health nursing, the target of care is the community, thus the community is the client receiving the care. The role of the nurse is to evaluate health concerns and develop an aggregate plan of care to address those concerns. Aggregates or target populations in the community may include child care centers, cities, counties, senior centers, homeless shelters, minority communities, faith-based organizations, work sites, schools, or other populations.
Identify and discuss a few targeted populations in your community that are of interest to you.

In my community, I would be interested in targeting populations such as older adults and homeless individuals. Older adults are a growing population and often have unique healthcare needs related to aging, chronic conditions, and social isolation. Homeless individuals also have complex healthcare needs related to physical and mental health, substance abuse, and lack of access to healthcare. Targeting these populations would require a comprehensive approach that includes collaboration with other healthcare providers, community organizations, and policymakers to address the social determinants of health that impact these populations.
One potential rationale for targeting older adults is that they are at increased risk for chronic conditions, such as diabetes, heart disease, and cancer. By targeting this population, we can focus on prevention, early detection, and management of these conditions. Additionally, older adults may also experience social isolation, which can negatively impact their physical and mental health. By providing targeted interventions, such as social activities, counseling, and education, we can improve the overall health and well-being of this population.
Similarly, targeting homeless individuals can have a significant impact on their health outcomes. Homeless individuals are at increased risk for communicable diseases, substance abuse, and mental health disorders. By providing targeted interventions, such as outreach services, case management, and healthcare access, we can improve the health and well-being of this population. Additionally, addressing the social determinants of health, such as housing, education, and employment, can have a long-term impact on the health of homeless individuals.
Overall, targeting these populations in my community can help to address the unique healthcare needs of these populations and improve their overall health and well-being. By collaborating with other healthcare providers, community organizations, and policymakers, we can develop comprehensive approaches that address the social determinants of health and improve health outcomes for these populations.
