Clinical Pathway/Case Management
For this paper I would like to write about disease management of testosterone deficiency syndrome, also known as late-onset hypogonadism. I am a nurse practitioner student my clinical preceptor is an integrative health nurse practitioner (woman). My preceptor treats low testosterone by taking patients testosterones levels and treating with weekly injections of testosterone. She teaches the patients how to safely administer on their own, but follows up with them in person to go over their most recent labs monthly. It costs the patients about $200 a month for the injections as well as the labs. She allows the patients to spread this out over 4 weeks or a monthly payment. Clinical Pathway/Case Management
In consultation with the clinical preceptor, identify aspects of case management, disease management, and/or population health as they relate to the selected clinical agency.
Submit a paper that is no more than 4 pages excluding title and reference pages. The paper should follow APA 6th edition guidelines and follow the writing rubric provided in the Course Resource section of the course.
Include the following information:
A. Identify a disease process that is common to the population served by the selected clinical agency.
B. Determine if the selected clinical agency has a case management, disease management, and/or population health approach to this issue. What are the processes in place within the healthcare organization to manage the care of individuals and population groups with this disease process? What are the processes in place to provide population based care management as it relates to the disease process?
C. Locate a clinical pathway designed to address the selected disease process either at the clinical agency or available through an internet or literature search. Identify the source of the pathway and attach a copy to the paper. (Please include.)
D. Discuss the application of the selected pathway to the care of an individual client.
• Identify potential causes of variances from the pathway.
• Identify corrective measures that could be implemented related to the variances.
• Describe potential services that would be needed after discharge. How is this information included in the case management process?
• What is the potential impact on cost effectiveness of the selected pathway?
