SIGNATURE ASSIGNMENT PART ONE: PROPOSAL – DUE WEEK 11
Two of the four key messages from the 2011 IOM Landmark Report: The Future of Nursing: Leading Change, Advancing Health are:
• Nurses should practice to the full extent of their education and training
• Nurses should be full partners, with physicians and other health professionals, in redesigning health care in the United States
The transformation of healthcare provides nurses with the opportunity to practice to the full extent of their license. Keeping this in mind, you are planning to open a nurse led ambulatory clinic.
• Identify a community in need of healthcare, a target population, a payor structure, and services to be provided.
• Using the NY State Scope of Practice for NPs or DNPs https://www.aanp.org/advocacy/new-york develop a business plan for this nurse led ambulatory clinic.

Key Elements of a Business Plan
• Introduction and purpose/need for your clinic – Community identification including social determinants for health of the community
• Description of the business model you will use with market and competition analysis for the community. The clinic must be Nurse-Led therefore, you need to include details about the New York State Scope of Practice for NPs or DNPs
• Operational Plan – SWOT analysis or gap analysis for your community needs
• Marketing Plan – your unique selling proposition (as nurses), “The Why”
• Organizational Plan – who are you? What do you do?
