Zolpidem (Ambien)

Zolpidem is an approved short-term treatment for insomnia that does not contain benzodiazepines (Bouchette et al., 2022). It is on the American Geriatrics Society’s 2019 Updated Criteria for Potentially Inappropriate Medication Use in Older Adults list because it can have the same bad effects on older people as benzodiazepines, such as delirium, falls, and a higher chance of needing to go to the hospital. From what I know, this medicine was given to older people in the hospital with care. Due to the possible side effects, this wasn’t a first-choice sleep aid. I do remember that after taking this, some of our patients “slept walked” or became delirious, and the next morning they had no idea what they had done. We were lucky to be there to watch and help, but if we hadn’t been, they could have gotten hurt.

Absorption

Zolpidem is quickly taken in by the body and has a short half-life in healthy people (Bouchette et al., 2022). People get less mobile as they get older. The RCTs showed that this group’s concentrations were higher, so they should only get the lowest dose of 5 mg (Bouchette et al., 2022).

Metabolism

This medicine is changed into a metabolite that doesn’t work. Hepatic damage, especially in older people, can change how a drug works and how much of it is given (Bouchette et al., 2022).

Distribution

This drug is found in many parts of the body and is able to get into the brain. zolpidem can also bring back brain function in people who are in a vegetative state because of a brain injury (Bouchette et al., 2022).

Excretion

Zolpidem leaves the body through the kidneys (Bouchette et al., 2022). The older people will have less clearance.
