The unapproved use of approved drugs, also called off-label use, with children is quite common. This is because pediatric dosage guidelines are typically unavailable, since very few drugs have been specifically researched and tested with children.

When treating children, prescribers often adjust dosages approved for adults to accommodate a child’s weight. However, children are not just “smaller” adults. Adults and children process and respond to drugs differently in their absorption, distribution, metabolism, and excretion.

Off-Label Drug Use in PediatricsOff-label drugs Pediatric Off-Label Drug Use In pediatrics, off-label drugs are used when there is a lack of effective and approved medication to treat a specific condition. Due to the availability of evidence from clinical trials about the effectiveness of medication, healthcare practitioners may also prescribe off-label drugs (Moulis, Durrieu & Lapeyre-Mestre, 2018). Amoxicillin, for example, is used to treat ear and chest infections in children. The drug is widely used due to the availability of research evidence demonstrating the medication's effectiveness and lack of side effects (Moulis, Durrieu & Lapeyre-Mestre, 2018). Another reason is a lack of clinical trials because researchers primarily focus on adults rather than children under a certain age. Off-label drugs have a certain percentage of health risks that clinicians must effectively manage. One method of ensuring a child's or infant's safety is to rely on available evidence (De Zen, Marchetti, Barbi & Benini, 2018). Peer-reviewed sources have indicated that various drugs and dosages are safe. A practitioner can also confirm a drug's safety using the FDA Medwatch program. Consensus statements and AAP guidelines are two other reliable sources of information about the safety of off-label drugs (De Zen, Marchetti, Barbi & Benini, 2018). Other measures that clinicians can take include checking a child's weight, medical history, and other complications. The data will aid in the safe prescription of off-label medications. Off-label drugs necessitate extra caution in order to avoid complications. One of the medications is morphine, which is used to treat pain in children. Because the drug can cause complications such as breathing problems, it must be used with caution (Gore, Chugh, Tripathi, Lhamo & Gautam, 2017). Cyclizine should also be used with caution in children because it can cause blurred vision.
