Bobby Randall, a 67 yo male stubbed his left big toe 6 days ago & is requesting Vicodin for pain. He has a history of HTN, DM, chronic foot pain, a history of Hepatitis C & is a former smoker & IV drug user. He is taking Lisinopril, Metformin, Celexa, & has been treated for his Hep C by a GI specialist. He has NKDA. Your foot assessment indicates he has no signs of infection and has mild swelling with slight tenderness on palpation at the hallux metatarsal phalangeal joint. His blood pressure has been running around 152/88 in the past 2 weeks. You calculate his 10 year ASCVD risk to be 16%. He is taking Lisinopril, Metformin, and Celexa, and a GI specialist has helped him with his Hep C. NKDA is his. Your Assessment of his foot shows that there are no signs of infection and that the joint where the big toe, second toe, and third toe meet is mildly swollen and slightly painful to touch. In the last two weeks, his blood pressure has been around 152/88. You think that his risk of ASCVD in 10 years is 16%.
